Limited liability companies file Form L-122, the Certificate of Change. Corporations and limited partnerships file Form C-104G. Both carry a $25 state filing fee, and you can submit by mail, in person, or online through the Division of Revenue's business amendments service. If your annual report is due, you can also change agents within that filing for the same fee.
